Apparently, the Menezes shooting could happen again.

But they've already shot him once, fatally, for God's sake. A second shooting seems redundant. Especially as he was apparently innocent, anyway. Poor guy.

I'm going to comment on my own thing here.

What mistakes are considered inevitable?

Apparently this:

"Police marksmen shot Mr de Menezes, 27, seven times in the head at point-blank range on a train carriage on July 22, 2005."

Christ, imagine if I, or you, before we went out to work, thought to ourselves, well, regrettable as it is, I just don't think I can avoid shooting an innocent person seven times in the head at point-blank range today. Or, for instance, well, regrettable as it is, I just don't think I can avoid shooting Steve Swain seven times in the head at point-blank range today. Oh dear. It's a difficult job, but somebody has to do it.
